Report a Lost or Stolen Card

If your library card is lost or stolen, you can notify us through this form so we can disable the card to prevent unauthorized use. You can also report lost or stolen cards by calling or stopping in at any of our locations. Until you receive confirmation that your card has been disabled, you are responsible for any materials checked out on your card.

Submitting this form will not automatically result in you receiving a replacement library card. As soon as possible, please bring identification with you to one of our libraries to get a new card.
